Instructions
Step 1:
- Log in to your account at system.gosport.com:
Step 2:
- Toggle to your child's account by clicking on the family and then click on your child's name:
Step 3:
- From within the player account, click on "Forms":
Step 4:
- Under available forms you will see each of the OSA forms and be able to click the start button for each of these to complete them:
