How to Complete OSA Player Registration Waiver

Once a player has been attached to a club that is affiliated with Ohio Soccer Association, they'll be able to access any required forms and complete them. This article will provide you with a step-by-step process of how to complete OSA Player Registration Waiver.

Instructions:

Step 1:

  • Login to your account at system.gotsport.com

Step 2:

  • Toggle to your child's account by clicking on the Family tab, and then click on your child's name

Step 3:

  • From within the player account, click on Forms

Step 4:

  • Under Available Forms you will see each of the OSA forms. Click the Start button next to OSA Player Registration Waiver to complete the form.
