As an Admin - Adding/Creating A Medical Release Document Repository
A document repository acts as a folder that stores all of the signed forms for this type of document. For example, if you had a club waiver, instead of adding it into a program as a custom question, you can add a "Club Waiver" repository and will then add that form to all of your programs. Follow the steps below to set up a document repository.
Instructions:
Step 1:
- Click the "Forms" dropdown in the Dashboard menu and select "Forms".

Step 2:
- Click "New" to create a form.

Step 3:
- Build out your form.
NOTE: If you are using GotSport for registering players, you can make this form "Open" and it will automatically be included within all of your programs. Once a player completes the form, they will not be asked to complete the form again during future program registrations with your club.


- Make sure to make the type "Medical Release". Once finished, scroll to the bottom and click "Save".

Step 4:
- Click "Registration Form" at the top of your screen.

Step 5:
- Click "New Form Element" to add questions to your form.

Step 6:
- Build out your form
- Make sure to make it "File Field" and "Required".

**Note that as a club, you would not need to create this form if your state association already requires a medical release form from all users with the player role.
