US Club Soccer: As a Club Admin - How to Submit R-002 Form 

This article will guide a club admin, affiliated with US Club Soccer, to submit R-002 information in bulk for players that have submitted hard copies. 

Instructions:

Step 1:

Step 2:

  • Click on Club Management.

Step 3:

  • Click on Players.

Step 4:

  • Select the players you would like to certify as having the R-002 form on file.

Step 5:

  • Click Create Form Registrations.

Step 6:

  • Choose the R-002 form listed in the dropdown.

Step 7:

  • Click Save.
