# How to Submit the Required Event Form for Your Player
## This article will demonstrate how a parent can fill out and complete a required waiver for their player. Please follow through and read these instructions carefully as the waiver will need to be completed correctly for your player to be approved on a roster.
If you are only a Parent of a player, and not a Manager/Coach, or Club Admin, please reach out to your Manager/Coach if you have any questions regarding your accounts, userIDs, etc. They will have the access and ability to look and see if your account already exists in GotSport. Creating incorrect duplicate accounts in GotSport will result in complications.
**Instructions:**
**Step 1.**
- You **MUST** be signed into your parent account in GotSport and have your Date of Birth updated. The waiver requires a parent/guardian over the age of 18 to sign and complete the waiver. **Please do not sign directly into your child's account.**
*NOTE\*\* You can click the "**Create** **an** **Account**" button below the Log In button, which will prompt you to create an account. If you get a UserID already exists error message, you can use the Forgot My Password tool.*
**
**Step 2.**
- After signing into your Parent account, please select the **CHILD** that is participating in the event. If you have multiple players participating, you will need to submit the waiver per child.
- Or, if you don't see your child listed, please click the **SEARCH** button
**Step 3:**
- If you were able to select your child, you should be all set to complete the waiver.
- If you clicked the **Search** button, a new window will have appeared.
- Fill out the information for your child and click Search.
- If you are confident your player exists in GotSport, but get a "**No User Found**" message, **DO NOT CREATE A NEW PLAYER.**
- Instead, try searching for other aliases for your player. For example, Steve vs Steven will be treated as different accounts.
- *NOTE\*\* If you are STILL not able to locate your player, please contact your Manager/Coach for the team, or your Club/Organizational Admin for help. Creating a New Player that is not connected to the Event Roster will not satisfy the requirement to be approved on the roster. The waiver MUST be completed on the players account that is on the event roster.*
