How to Locate/Complete MVYSL Forms

This article will demonstrate how a parent or Manager/Coach can locate and complete any forms required by MVYSL

Step 1:
  • Log in to your account at system.gotsport.com

Step 2:

  • Toggle to your child's account by clicking on the Family tab and then click on your child's name.
  • **Note: If you are a Manager/Coach, skip to Step 5.

Step 3:

  • Now within the player account, click on the Forms tab.

Step 4:

  • Under Available Forms, click Start next to Miami Valley Youth Soccer League's Parent Code of Conduct form.

Step 5:

  • If you are a coach, after logging in, click on the Dashboard tab.

Step 6:

  • Click on Available Forms and then click Start next to MVYSL's Coaches Code of Conduct to complete the form.
