Instructions:
Step 1:
- Login to your account at system.gotsport.com
Step 2:
- Toggle to your child's account by clicking on the Family tab, and then click on your child's name
Step 3:
- From within the player account, click on Forms
Step 4:
- Under Available Forms you will see each of the OSA forms. Click the Start button next to OSA Player Concussion Info Sheet to complete the form.
