# As an Admin – How to Submit and/or Approve the US Club Soccer Player Loan Form
## Learn how to submit a player loan request as the Receiving Organization, and how to approve or deny one as the Sending Organization.
The US Club Soccer Player Loan Form is used to temporarily loan a player registered with US Club Soccer from one affiliated club to another for a specific competition. The Receiving Organization initiates the request through GotSport, and the Sending Organization reviews and approves or denies it.
***Note -** The player must have a current, valid US Club Soccer member registration with the Sending Organization before this form is submitted.*
***Warning -** Approval from US Club Soccer is not required. GotSport may display the form as "US Club Soccer approved," but this is automatic and does not mean it was reviewed by US Club Soccer. The player loan is only valid and insurance coverage only applies when: (1) the player has a current, valid US Club Soccer registration with the Sending Organization; (2) both organizations have approved the form; and (3) the player participates in US Club Soccer-sanctioned and -approved programming.*
### **Part 1: Submitting the Form (Receiving Organization)**
1. From your Club Dashboard, go to **Affiliations** \> **US Club Soccer** \> **Available Forms** tab, then locate and open the Player Loan Form.
2. Select the Receiving Organization, the Sending Organization, and the player role, then click **Continue**.
3. Fill out all required form fields, then sign your name in the signature field.
4. Once submitted, a green success banner will appear confirming the form has been sent to the Sending Organization for review.
### **Part 2: Approving or Denying the Form (Sending Organization)**
1. From the **Club Management** menu, go to **Forms and Requests**.
2. Use the filter to search for and locate the Player Loan Form submitted by the Receiving Organization.
3. Click the form's ID to open it.
4. Click the **green check mark** on the right side of the form to approve the request, or click the **red X** to deny it.
### **Part 3: Retrieving the Approved PDF (Receiving Organization)**
1. From the **Club Management** menu, go to **Forms and Requests**.
2. Use the filter to find the approved Player Loan Form.
3. Click the **three dots** on the right side of the form and select **View PDF**. From the PDF view, print a copy to present at the competition.
***Note\*\*** The Receiving Organization must present both the printed, fully approved PDF copy of this form and the player's passcard (provided by the Sending Organization) at the competition.*
