As a Parent/Player - How to Complete the R-002 Form from US Club Soccer

This article will demonstrate how a Parent can sign into their GotSport account to complete the US Club Soccer R-002 Form. This form must be completed in order for the Player to meet their Risk Management requirements.

Instructions:

Step 1: 

  • Sign into your Parent account and click Family
  • Click on the name of the Player whose profile you'd like to view.


Step 2:

  • Now you'll be viewing the player profile, click Dashboard to find the requirements.

Step 3:

  • Scroll down until you see the Risk Management Requirement and click Details next to the R-002 Form.

  • Additionally, you can also locate the form by clicking the "Forms" tab at the top of the player profile.


Step 4: 

  • Complete the Form
  • Be sure to select the PLAYER account you're filling the form out for, not your name.
